Terms and conditions and rules of the AZ card loyalty program

1.

The operator of the AZ card loyalty program is AZ Catering services s.r.o., Company ID 26395916, with its registered office at Sladovnická 4, 326 00 Pilsen, entered in the Commercial Register kept by the Regional Court in Pilsen, Section C, File 17620 (hereinafter referred to as the “Company”).

The Company reserves the right to change and amend the rules of the AZ card loyalty program (hereinafter referred to as the “Rules”) and the amount and form of all bonuses to it at any time.

3.

Only loyalty card holders are entitled to benefit from the benefits of the AZ card loyalty program. Any natural person who requests the issuance of a card and agrees to the Rules becomes the holder of the AZ card loyalty card.

4.

The loyalty card will be issued to the customer by the staff of selected establishments of the “U nás Plzeň chútná” chain listed below. The issuance of the loyalty card is free of charge. In the event of loss or theft of the loyalty card, the issuance of a new card and the transfer of the points balance will be charged CZK 25. To take advantage of all the benefits of the AZ card, the customer has the option of filling out the registration form at www.unasplzenchutna.cz/en/az-card/. By filling out the registration form, the customer will receive additional benefits of the loyalty program, and is also protected in the event of AZ card status issues. This online registration form can also be filled out after previously using the AZ card in any of the “U nás Plzeň chutna” establishments.

Other benefits resulting from registration:

- a birthday gift from the “U nás Plzeň chutna” establishments.

This gift can only be used if the AZ card user has registered correctly in the system. The birthday gift can be redeemed once, only on the loyalty card holder's birthday, in person at one of the "U nás Plzeň chůntů" establishments. The birthday gift redemption is subject to further consumption.

- possibility of blocking the card and saving the balance

The holder can immediately use the loyalty card (the card is immediately active upon the first purchase) for their purchases at the following establishments:

Šenk Na Parkánu, Veleslavínova 4, Pilsen

Restaurate U Salzmannů, Pražská 8, Pilsen

Restaurant 12, Kopeckého Sady 8, Pilsen

Osteria Martinská, Martinská 7, Pilsen

Osteria Garage, Částkova 56, Pilsen

5.

With every purchase in one of the above-mentioned "U nás Plzeň chůtně" establishments, upon presentation of a loyalty card, the establishment's staff will load the loyalty card to the given account to obtain a bonus of 10% of the spending. Bonuses are always added to the card account the following day.

The bonus can be obtained for the purchase of all products from "U nás Plzeň chůntá" establishments, except for tobacco products and lunch offers.

6.

The customer is entitled to receive a bonus when paying in any form - cash, meal vouchers, gift voucher.

7.

The loyalty card holder can use the accumulated bonus points when paying for "U nás Plzeň chůntá" products in selected "U nás Plzeň chůntá" establishments listed in point 4 of these Rules. It is not possible to obtain cash for bonus points.

8.

When claiming a discount on a purchase (e.g. by using a discount card or a discount offer), the bonus is not credited.

9.

The current balance of bonuses will be communicated to the loyalty card holder upon request by the staff of any "U nás Plzeň chútná" establishment listed in point 4 of these Rules.

10.

In the event of a technical malfunction of the payment terminal, the Company is not responsible for the system's malfunction or for a delay in crediting bonus points. The loyalty card holder has no legal right to load bonus points.

11.

The validity of the loyalty card and bonus points is unlimited.

12.

The loyalty card holder is obliged to notify the company of the loss or theft of the loyalty card without delay at the email address info@azcatering.eu. The company will ensure the deactivation of the lost or stolen card and the transfer of bonuses to a new loyalty card.
